Medicaid Redesign Team (MRT) Proposal #191

• 9-month project
• Under the 2010-2011 state budget appropriation
• New York State Department of Health (DOH) contract with the School of Public Health
• Recruit 4 regional Collaboratives (Each with cross-setting partners – hospital, nursing home, home care at minimum) – Buffalo, Rochester, Kingston, Binghamton
• Facilitate implementation of Gold STAMP – onsite coaching/training supports
• Measure process changes and impact on pressure ulcer
